Proof of Life

A proof of life is a written statement confirming that you are alive. Pension funds, life insurance companies, or other benefit-paying agencies may request this. Apply for the proof online using DigiD or make an appointment. For foreign agencies, you will need an Attestatie de vita . You must always apply for this in person at the service counter. Please make an appointment to do so.

Bring to the appointment

  • The original document;
  • A valid form of identification;
  • If applicable: a written authorization and a copy of a valid form of identification from the principal.

Authorization

You may authorize someone to pick up a life certificate on your behalf. The authorized representative must bring a written authorization signed by you, along with a copy of your valid identification and his or her original, valid identification.

What are the costs?

  • At the counter: €8.25
  • Online application: €5.60
  • This document is free of charge if you provide proof that you need it for pension purposes.
